I understand that submitting this form only places me on a waitlist, but it does not establish care or create a coach-client relationship, guarantee an appointment or guarantee that I will be contacted. I understand that I may be contacted if or when an opening becomes available, contingent on capacity.

I understand that coaching is not a clinical service and is not insurance reimbursible.

I agree to call emergency and/or crisis services in my local area and understand that form submission through this coaching website are not a place to route emergency content. I understand that Silhouette Coaching asks that I not enter any urgent, emergent, or highly sensitive personal or medical information into this form. I agree to honor this request.

I confirm that I am over the age of 18.
